Data Protection Act 1998 GDPR 2018: How we use SERVICE USER information

We process personal data relating to those we support as part of our Health and Social Care regulated activity.  We do this is for knowledge purposes, to assist in supporting the people we support.

The collection of this information will also benefit the people we support by:

  • Improving the quality of data across the sector

The personal data includes identifiers such as Name, Date of Birth, Address, Personal characteristics such as gender, diagnosis, medication

We will hold this information in the form of:

Care plans/risk assessments/health action plans

Photographs

Reviewing information

Medication Folder/MAR sheets

Appointment recording

We will also hold PII for the people we support’s next of kin

This list is not exhaustive and may change to meet the needs of the people we support 

We will not share information about you with third parties without your consent unless the law allows or requires us to.
